A personal transport budget is money given to you to pay for travel to and from school, for a child with special educational needs.
If you have been told by us that we will give you travel assistance through a personal transport budget, you will need to complete this form to set up an account.
You will need
Latest bank statement - showing your full name and address
Name of bank account holder
Name and full postal address of your bank or building society
Bank or building society account number and sort code.
The proof of address and bank details need to belong to the person applying.
Uploading supporting documents
We will ask you to upload some documents. For example, a document showing proof of address, latest bank statement.
Accepted file types: Word (docx), Acrobat (pdf), Excel (xlsx), Plain text (txt)
Zip files can't be uploaded
We also accept photos taken on phones and tablets and the following image file types - jpeg, gif, png
There is a maximum individual file size per document of 50MB.
What happens next
We aim to set up your account within 4-8 weeks.
Once we have set up your account, the payment will be processed in the next payment run. This takes place every four weeks. If you would like to get a timetable of when you will get your payments, email email@example.com.
